Express Cardio-Metabolic Check and Frailty Rehabilitation for Older Adults Attending Elderly Centres

Offer rapid access to cardiac examinations, advanced blood analysis for metabolic and nutritional assessment, and interventions related to frailty and pre-frailty for grassroots elderly

Social Problem

  • Ageing, poverty and social inequity as determinants of reduced access to healthcare, increased vulnerability to non-communicable diseases and frailty, and increased cardiovascular disease burden

Social Innovation

  • The project combines the implementation of NMR metabolomic profiling, rehabilitation , and an exercise maintenance app
  • The collected data and information will be used to build a digital health hub focusing on health recovery and cardiovascular ageing

Expected Social Impact

  • 800 older adults will receive Express Heart Checks, blood analysis, and cardio-metabolic risk and nutritional assessments
  • At least 800 older adults will receive education through Public Seminar on cardiovascular disease, heart failure and frailty
  • 180 frail or pre-frail older adults will undergo a 12-week rehabilitation and exercise program
  • 800 frail or pre-frail older adults will be educated about or use an e-health exercise maintenance app
  • 40 individuals  will be identified with at least moderate grade cardiac ultrasound abnormalities, while 20 will be diagnosed with stroke-associated atrial fibrillation, and 20  will require anticoagulation to prevent stroke
  • A 10% to 15% improvement in frailty and physical fitness measures is expected among frail and pre-frail older adults undergoing rehabilitation
