Social Problem

  • Caregiver Burden: Family caregivers of persons with disabilities face chronic emotional/physical strain, exacerbated by limited support systems and high burnout risks.
  • Transition Gap: Post-school service discontinuities lead to skill regression and social isolation for special needs graduates.
  • Systemic Fragmentation: Critical shortages in specialized training, emotional support, and community integration mechanisms perpetuate dependency cycles.

Social Innovation

  • Family Involvement: Integrates family-based support, offering caregivers mindfulness stress reduction and life review training alongside disabled youth’s life skills like shopping, gardening, and woodworking.
  • Art & Nature Therapy: Uses art sessions and nature experiences to promote mental and physical well-being.
  • Community Empowerment Model: Creates caregiver peer groups and volunteer-matching to build a sustainable support network and reduce isolation.

Expected Social Impact

  • Organise 24 thematic talks on caregiving skills for 120 caregivers of special school leavers
  • Organise 10 online/offline meditation groups for 80 caregivers
  • Establish 10 mutual support groups
  • Provide 253 sessions of life skills training for 24 special school graduates
  • Arrange 8 sessions of talks for 50 volunteers
  • Match 24 volunteers with special school graduates
