Service Types:

Social Problem

  • There is a lack of comprehension regarding the rationale behind prescribed medications and medication labels, which are only available in Chinese and English. This lack of understanding hampers proper usage and adherence to medication regimens
  • This makes effective medication management crucial, but unfortunately, it is often poor, leading to low adherence to treatment plans and subsequent health complications
  • During transitions between healthcare providers, proper medicines reconciliation is essential. However, this process is often overlooked, leading to medication errors or omissions that can have serious consequences for patients
  • The duplication of medicines received from both local and home countries, stemming from a lack of medical knowledge. Peer group influence, cultural beliefs and socioeconomic situation also contribute to suboptimal medication usage

Social Innovation

  • An innovative intervention: Multicultural digital pharmacy
  • Utilize the strength and resources of P&I community: forming multicultural healthcare support network
  • Collection of medication errors and interventions data in multicultural communities

Expected Social Impact
